What’s the issue

Sometimes we make mistakes. When that happens, how do you respond?  How do you ensure the mistake doesn’t happen again?

In large organizations, you cannot control every step of a process. When an error is made, do you blame the people or the process.  I’m learning the process may be to blame. Processes enable people to shift responsibility to others and never take full ownership of a decision.  Is it time to strip down processes to allow employees to learn and take control?
